Septoplasty / Deviated Septum Correction During Rhinoplasty

Many patients seeking rhinoplasty may also suffer from some degree of nasal obstruction. This may be congenital or due to some sort of trauma. Some of the causes of this include a deviated septum, enlarged turbinates, inflamed mucosal tissues due to allergies or pollution or a combination of any of these factors.

Sometimes nasal obstruction can be improved non-surgically with inhaled nasal steroid sprays while other times, adjustments to the septum or turbinates are required such as septoplasty, turbinectomy or turbinotomies (outfracturing of the turbinates).

Techniques for Rhinoplasty

Techniques for rhinoplasty have evolved quite a bit compared to the rhinoplasty techniques utilized in the 1970s and 1980s when a cookie cutter, closed rhinoplasty technique was utilized to create a small, pinched and non-functional nose.

When To Use Open Rhinoplasty

During his Orange County nose job surgeries, Dr. Grover often utilizes an open rhinoplasty technique when a patient requires a fair amount of work on their tip. The open rhinoplasty technique, which utilizes incisions inside the nose and a very small, imperceptible incision on the underside or columella of the nose to lift up the soft tissues or skin, allows Dr. Grover to directly see the underlying framework of cartilage and bone. This technique allows Dr. Grover to make precise adjustments and modifications to these structures to create a more sculpted result which is well supported to last a long period of time. The open technique allows for greater precision as it enables the Dr. Grover to precisely measure the remaining cartilage with measuring tools such as a caliper or ruler.

Anesthesia During Rhinoplasty

Rhinoplasty may be performed under general anesthesia, twilight anesthesia or even local anesthesia with oral sedation; however, Dr. Grover often performs his Newport Beach rhinoplasty procedures under general anesthesia with the assistance of board-certified anesthesiologists. Most of Dr. Grover’s patients experience minimal bruising and minimal discomfort and are off pain medications the day following cosmetic surgery. Nasal packing is not utilized and intranasal dissolvable sutures are used instead. An external nasal splint is placed on the nose following rhinoplasty and is removed within 5-7 days in the office. A patient’s nose will appear beautified immediately when the splint is removed although some swelling will be present. Most of the swelling resolves within the first 3-4 weeks, but takes 12-24 months for it to full resolution. Night taping may be utilized during the first month following surgery to help reduce swelling.

What is rhinoplasty?

A rhinoplasty is usually referred to as a “nose job.” It involves a range of surgical procedures that reshape the nose structure for aesthetic or functional reasons. Rhinoplasty is most often sought to resolve an aesthetic issue, such as an over-large nose, a large hump on the nose bridge, a drooping tip, an asymmetrical nose, overlarge nostrils, and/or a bulbous tip.

Is rhinoplasty surgery safe?

Any surgery carries some risk, but it is rare for a patient to experience complications from a rhinoplasty. One issue of great importance is that the procedure is performed correctly – the first time – to help you avoid the need for a revision surgery to correct a bad outcome. How long will I need to recover?

Most patients take two weeks off work. During the initial stages of healing you will be wearing a splint to keep the revised nose structure protected and in place. Swelling is usually greatly reduced and no longer visible to others after about two months, after which your nose will continue to improve and take on the final, sculpted, more attractive appearance.

How old must you be to have a nose job?

As the nose continues to grow until a person reaches about fourteen for girls, and slightly later for boys, it is advised that the surgery is performed after that point. This also allows the surgeon to see the patient’s adult facial features and create the most balanced look to enhance the overall facial appearance.

What type of anesthetic will be needed for a nose job?

Patients may be under general anesthetic, or some may have local anesthetic used in conjunction with sedation.

How long is the rhinoplasty procedure?

Will I have visible scars after a nose job?

An open rhinoplasty involves an incision beneath the tip of the nose. This surgical technique leaves a very well hidden, fine line, that fades over time to become virtually invisible.

Will I be in pain after a rhinoplasty?

When the surgery is complete, you will still be experiencing the effect of the anesthetic. Once this wears off, there will typically be some discomfort. To manage any pain, we provide a prescription for pain medication, so you are comfortable in the initial healing stages.

Can I drive myself home after the procedure?

Arrange a friend or family member to transport you home. You will feel groggy and tired, and it is not safe to drive. Have that person help you for the rest of the day and evening. Your only job will be to rest and recover from surgery.

How long will I need to take off work after a rhinoplasty?

Most patients return to work one to two weeks after the procedure. If you can, plan for a full two weeks off.
